HOME CARE PAGKAGES | Support at Home Program
We can help you access the in-home support you need.
The way the Australian Government provides funding for in-home care is changing. The current Home Care Package (HCP) program will become the new Support at Home Program from 1 July 2025. This government-subsidised funding allows you to remain living independently in your own home for longer, with the support you need to manage everyday tasks and maintain your well-being. The new program remains flexible and person-centred, meaning you continue to have choice and control over the services you receive and who provides them.
We understand the complexities of the system, and we’re here to provide additional support.
How we can help you
- Understand your Home Care Package / Support at Home Program options.
- Develop a personalised care plan that maximises your package funding.
- Coordinate your services and ensure you receive high-quality care.
- Adjust your services as your needs change.

Your partner in Support at Home Program management.
Using your Home Care Package with us is simple, and we are fully prepared for the program's transition to the new Support at Home Program from 1 July 2025. The process starts with a free, no-obligation consultation to discuss your needs and goals. We'll then review your current funding agreement, working with you to create a personalised care plan that aligns with your assessed needs and preferences. Once you're happy with the plan, we'll create a clear Service Agreement and match you with a compatible carer from our team. Your services will then begin, and we'll provide ongoing support and regular check-ins, ensuring you receive the best care both now and under the new program. We are here to help.
